ADULT RAPID REHOUSING

Five Keys Adult Rapid Rehousing Programs offers case management and supportive services for a variety of programs in San Francisco that link unhoused clients to permanent housing and supportive housing, using housing first and harm reduction principles. Case managers and supervisors ensure through a rigorous process that participants are on the path to economic and overall stability.
These programs are for individuals who are 18 years of age or older (without minor children) who are a part of the unhoused population.
Our case managers collaborate with participants to create and maintain a Housing Stability Plan that supports the participant to:
• Increase their income and employability
• Improve their credit history and rental stability
• Address behavioral health issues that negatively impact housing stability
• Identify and have access to permanent affordable housing
EMERGENCY HOUSING VOUCHERS
SAN FRANCISCO
Five Keys Emergency Housing Vouchers is a program for both transitional age youth (TAY) and adults. San Francisco’s Department of Homelessness and Supportive Housing is responsible for referring eligible families to the Housing Authority for this program. Participants typically pay 30% of their income on rent, with the rest covered by the Housing Authority through the voucher.
Emergency Stabilization Units

In partnership with DPH Behavioral Health Services Emergency Stabilization Unit, Five Keys supports individuals experiencing homelessness and suffering from behavioral issues to find stability and well-being while living at various SRO’s throughout San Francisco.
FLEXIBLE SUBSIDY VOUCHERS

Five Keys Flexible Subsidy Vouchers is a permanent subsidy program where clients can graduate out of the program for case management when they are able to live independently. After a participant has been in their new home for 3 months, if the household has an income, they begin to pay 30% of their total household income towards rent.
TAY RAPID REHOUSING
SAN FRANCISCO
Five Keys Rising Up Program provides rental assistance and supportive services to youth ages 18-25, who were previously incarcerated or are currently unhoused, by providing a time-limited housing program. Case managers utilize a network of community-based organizations, to support unhoused youth to receive housing location services, then provide wraparound housing focused case management and housing coordination services so young people can grow their income and maintain their housing. Rising Up is a public-private partnership, partially funded by SF Housing and Homeless Services through a sub-contract with Larkin Street Youth Services.
Housing Community Supports Services
ALAMEDA
Funded by CalAIM Housing Community Supportive Services and provides navigation and tenancy-sustaining services to the most vulnerable population in Alameda County.
