Pathways from Homelessness
to Stability

Five Keys Housing provides comprehensive case management and supportive services in San Francisco and Alameda counties, guiding unhoused individuals from homelessness to transitional and permanent housing. Using housing first and harm reduction principles, we help participants regain stability, dignity, and economic independence.

Our programs serve unhoused individuals 18 and older, offering:

  • Transitional Housing for short-term stability

  • Permanent Supportive Housing with long-term services

  • Housing Stability Plans to build financial and personal security

Our case managers work with participants to:

  • Increase income and employment opportunities

  • Build credit and rental stability

  • Address behavioral health challenges

  • Secure permanent, affordable housing

How to Get Someone Sheltered

While we are unable to accept direct self-referrals for shelter beds, the resources below can help start the process:

  • San Francisco: Call the San Francisco Homeless Outreach Team at (628) 652-8000 or visit the online portal to join the city’s shelter waitlist. 

  • Alameda County: Dial 2-1-1 to connect with emergency shelters and other support services in your area.
